"Ask for help. Not Because you are weak,
but because you want to remain strong."
but because you want to remain strong."

Community-Based Rehabilitation Service (CBRS) is a service that is done within the client's home and community. These services allow a client to build upon what they're learning in their counseling sessions and allow them to practice skills within a community setting. These services tend to be provided for 3-4 hours a week depending on client need.
Case Management (CM) helps you to access vital community resources to support you and your family where needed including housing, employment, food, children's items, clothing, transportation, healthcare, school services, etc.
CBRS & CM are currently only offered to Medicaid clients upon authorization. Talk to your counselor if you're interested in these services for yourself our your child.
